Once completed, print and/or email the checklist to help you acquire all needed documentation before going to a Deputy Registrar Agency to obtain your driver license or identification card.
To prove identity, you must show proof of the following five elements
1-Full Legal Name*; 2-Date of Birth; 3-Proof of U.S. Legal Presence; 4-Social Security Number; 5-Proof of Address
To prove an element, the element must be contained on the document. One document from this list may be presented for a duplicate or renewal issuance (does not apply to Non U.S. citizens).
